Submit a Complaint
The Data Privacy Ombuds serves as a resource for individuals who file a data privacy complaint and governmental entities which may be the subject of a complaint. The Ombuds may also mediate data privacy disputes between individuals and governmental entities. You may file a complaint about a governmental entity’s data privacy practices or request mediation of a data privacy dispute by filling out the form below. You must provide a detailed description of your data privacy concerns and your contact info so the Ombuds can reach out to you. You do not need to provide your name, but you should attach any relevant documents. If you fail to provide this information, the Ombuds may not be able to fully resolve your data privacy issues. The information you provide will be used by the Ombuds for the purpose of resolving your data privacy complaint or mediating your data privacy dispute. This information may be shared with the governmental entity that is the subject of the complaint, but your personal data will not be shared or sold.
